Newborn Membership

(Birth to 60 Days Old)

$2000

Comprehensive care during the first 60 days of life.

Includes 3 house calls during the first 60 days (First week, two weeks and 1 month of age).

Covers the first two months of care.

Monthly Membership Fee: $300/month after the initial 60 days.

(Over 60 Days Old)

$1,000 (one-time enrollment fee)

Monthly Membership Fee: $300/month for a single child.

If multiple children are enrolled, the fee is reduced to $250/month per child for all children.

Routine Well Visits and Sick Visits

Four visits per year are included annually, which can be used for routine well visits or sick (urgent care) visits. Routine well visits follow these schedules:

• First Year of Life: All well visits are used for well visits at 2 months, 4 months, 6 months, and 9 months.

• Second Year of Life: Three of the four visits are designated for well visits at 12 months, 15 months, and 18 months; the remaining one can be used for urgent care.

• Third Year of Life: Two of the four visits are designated for well visits at 24 months and 30 months; the remaining two can be used for urgent care.

• Age 3 and Above: One visit annually for a well visit, with up to three visits available for urgent care.

Urgent Care Services

Urgent care is available virtually or in-office

• After the four included visits are used, additional urgent care visits are available at the following rate:

1. In-office urgent care: $150 per visit (M-F 9a-5p)
2. All virtual care included (7 days/wk 8a-8p)

Email Access

• Routine questions are answered within 24 hours via email.

• I support families in making individualized decisions regarding vaccination schedules, including the option to delay or decline vaccines.

• Vaccines are available at-cost through a local pharmacy

• Personalized Attention: A low patient volume allows for truly personalized care.
